Connect you with food resources, CalFresh, and nutrition programs

Provide support with housing applications and community housing programs

Assist with transportation to medical appointments

Connect you to county and community social services

Link you to recovery programs, behavioral health services, and crisis support

• Hospital & Facility Transitions

Help you transition safely home after hospital discharge

Support you if you wish to return to the community after a stay in a nursing facility
