Provide direct skilled nursing care (e.g., wound care, medication administration, injections, vital signs monitoring) per RN plan

Implement and follow individualized care plans

Document observations, interventions, and patient responses accurately and timely

Report changes in patient condition to supervising RN or physician

Educate patients and families on care, medications, and safety

Assist with coordination of services and participate in team meetings

Maintain compliance with agency policies, CDPH, and Medicare standards

Travel to patient homes within the service area

Current, active Licensed Vocational Nurse (LVN) license in the State of California

Minimum 1 year of recent nursing experience (home health, acute care, or community health preferred)

Knowledge of home health care practices and regulations

Strong clinical skills, attention to detail, and communication abilities

Valid California driver's license, reliable vehicle, auto insurance, and willingness to travel

CPR certification (BLS) required

Home health experience and bilingual skills a plus
