Registered Nurse (RN) - Home Health Great Choice Home Health
We are seeking dedicated, licensed Registered Nurses to join our clinical team and provide skilled nursing care to our patients!

Employment Type: Full-Time / Part-Time / Per Visit (Flexible Scheduling Available)
Service Area: Los Angeles County, Orange County, San Bernardino County, Ventura County, Riverside County
Position Summary As an RN, you will provide intermittent skilled nursing services, develop and implement care plans, perform assessments (including OASIS), educate patients/families, and collaborate with the interdisciplinary team to ensure high-quality home health care.

Key Responsibilities

Conduct initial and ongoing patient assessments, including OASIS documentation

Develop, implement, and update individualized plans of care

Provide skilled nursing services (e.g., wound care, IV therapy, medication management, injections)

Monitor patient progress, report changes to physicians, and coordinate with other disciplines

Educate patients and families on disease management, medications, and self-care

Ensure compliance with CDPH, Medicare/Medi-Cal, and agency policies

Supervise LVNs and home health aides as needed

Complete timely clinical documentation and participate in case conferences
Qualifications
-
Current, active Registered Nurse (RN) license in the State of California
-
Minimum 1 year of recent nursing experience (home health preferred; acute care or community health acceptable)

Strong knowledge of home health regulations, OASIS, and Medicare Conditions of Participation

Excellent assessment, communication, and documentation skills

Proficiency with electronic health records (EHR) and Microsoft Office

Valid California driver's license, reliable vehicle, auto insurance, and willingness to travel within service area

CPR certification (BLS) required

Home health experience and bilingual skills (English/Spanish or other) a plus
